REGISTRATION/INFORMATION FORMS:
Registration forms are distributed to each student at the beginning of each school year or when a student transfers in from another school. Please provide as much contact information as possible so that a parent/guardian can be contacted in case of an emergency. If there are changes in work, cell, pager, or home numbers, please inform the health office ASAP at 624-6386.
MEDICATION POLICY/MEDICAL PROVIDER ORDERS:
The SCHOOL DISTRICT POLICY does not allow for the distribution of ANY medication, either prescription or over-the counter drugs, WITHOUT written consent from a medical provider. This policy includes lotions, creams, nasal sprays, extra-strength Tylenol, ibuprofen, eye drops, and correctly labeled prescription drugs (in their original pharmacy container). The medication must be kept in the Health Office at all times. Medical Provider orders must be renewed annually. Any medications not picked up prior to the last day of final exams in June will be destroyed. No medications will be stored over the summer. NOTE: Students with severe, potentially life-threatening conditions (ie. asthma and severe allergies) may be permitted to carry inhalers and Epipens. Written authorization from the medical provider must be provided annually and must state that the student is permitted to carry the medication on their person.
The Health Office can only distribute generic Tylenol (acetaminophen) if a parent/guardian has signed the school medication card that is distributed in homeroom at the beginning of each school year. REMEMBER, for each new school year a new medication order and new parent-signed Tylenol card must be submitted.
PHYSICAL EXAMS AND IMMUNIZATIONS:
According to the NH RSA 22:38, each student must have proof that their immunizations are up-to-date, along with proof of a physical exam. These forms need to be kept on file in the Health Office. If your student participates in athletics or the NJROTC program, a separate copy of each form will need to be submitted to the perspective offices. (None of these departments are able to exchange information.)
If, during the school year, you receive additional immunizations or have a physical examination, please be sure to submit this information to the Health Office.
Each student is required to receive the Tetanus-diphtheria booster vaccine every 10 years. Please request this information from your provider, which can be mailed, faxed, or hand-delivered by the parent/guardian or the student to the Health Office. Our fax number is: 628-6153.
HEALTH UPDATES:
If there are any significant health issues, emotional issues, medication additions, or changes, surgeries, or accidents that occur during the year, please inform the Health Office ASAP.
DISMISSALS:
The School Nurses have seen an increased number of students using their cell phone to contact their parents/guardians in order to dismiss themselves WITHOUT going to the Nurse. As you may be aware, cell phone use is STRICTLY PROHIBITED during school hours. The School Policy requires that the student MUST report to the Nurse when ill. The Nurse will then call the parent/guardian to arrange for dismissal from school. The main office WILL NOT dismiss students for reason of illness.
