The Manchester School District and Manchester Health District have implemented a new policy that requires all students entering the 9th grade to have a physical examination by a licensed medical provider. This new policy will be in effect beginning September 2007. Students will be excluded from school if they fail to provide documentation of a complete physical examination with one year prior to entry into 9th grade up to a six-month period after the 9th grade(March 1, 2008). This policy will provide an 18-month window of time to provide the required documentation for the students.
Upon completion of the examination, the student's medical provider should complete a physical examination form either provided by the practice or can be downloaded from this web site. Please send the complete form into the School Nurse. If your child does not have a medical provider or medical insurance and you would like assistance making an appointment, please call the Manchester Health Department at 624-6466, and ask for Health Link.
